- Throughout the Salt Lake City area,plete senior wellness visits atpany-owned outpatient facilities and independently contracted physician offices, as well as some in-home visits and in-hospital (transitional care) visits
- Up to 16 patients per day (10-12 patients per day on average)
- No prescribing or treatment, simply performing consultative assessments/evaluations
- Mileage Reimbursement
- Great work-life balance with no required weekends or nights
- Current, unrestricted APRN or PA License or ability to obtain within 90 days of employment
- Prescribing authority or ability to obtain prior to employment
- Current DEA certificate or ability to obtain prior to employment
- Board Certified as a Nurse Practitioner or Physician Assistant
- Must befortable working in various care environments such as Primary, Post-Acute, Acute and Home visits

- Family, Geriatrics or Adult medicine experience
- 1+ years of clinical experience in their highest level of education (met 2000 hrs. to practice autonomously)
- Proficiency with Microsoft Excel, Word, PowerPoint, Outlook
- Experience with HCC coding gelines in Primary Care
- 2 years of experience working in a managed care health plan environment
- Knowledge of Value Based Care Model
- HEDIS, Coding, Risk Adjustment knowledge

- Conduct Comprehensive Annual Wellness visits for Optum members at independent physician association (IPA) offices, skilled nursing/post-acute care facilities, home visits and potentially acute care hospital facilities
- Work within OptumCare proprietary EMR systems when conducting wellness visits
- Travel to locations as assigned
- Interface with physicians and/or medical group staff professionally developing credibility
- Other related duties as assigned and/or needed
- May need to attend future continuing education classes to keep abreast of medical advancements and innovative practice coding, Risk Adjustment gelines
- Promote the well-being of our senior members of their patients by conducting annual physical exam & wellness assessments with strong emphasis on senior conditions
- Document the senior assessment in an assessment form and a plan of care in a stand-alone medical record and proe a copy to IPA physician’s office at the end of day
- Review medications for polypharmacypliance and educating patients on medications
- Confirm, disagree or mark resolved chronic conditions from patient-specific lists of historical conditions; confirm plan of care developed for patients seen in the primary care clinic
- Complete a Collaborative review of chronic preventative measures for seniors for quality review
- Utilize tools integrated in the EMR to perform screenings (CORE, PHQ-9, 6-CIT, COA, etc.)
