*Job Summary* Full-time position available for a NY licensed OTR for this active sub-acute/LTC facility. Knowledge of care plans, MDS, Medicare/Medicaid regulations a must. Work in a warm and friendly environment and be a part of our dynamic team. Key Responsibilities & Duties: * *Patient Evaluation:* Assess physical, cognitive, and emotional abilities to determine functional levels and needs. * *Treatment Planning:* Develop, implement, and oversee personalized plans to improve self-care, mobility, and cognitive function. * *Therapeutic Interventions:* Provide direct therapy (strengthening, cognitive exercises) and teach compensatory techniques. * *Adaptive Equipment:* Recommend, provide, and train residents on using assistive devices (canes, walkers, specialized utensils). * *Environmental Modification:* Identify and suggest changes to resident rooms/facility areas for better safety and accessibility (e.g., grab bars). * *Fall Prevention:* Educate residents and staff on safe movement and hazard reduction. * *Cognitive & Social Support:* Engage residents in meaningful activities and tailor therapy for dementia/Alzheimer's care. * *Documentation:* Maintain detailed clinical and progress notes and contribute to comprehensive care plans. * *Team Collaboration:* Work with nurses, doctors, and other staff to integrate OT goals into overall care. * *Training:* Provide education to residents, families, and nursing staff on techniques and equipment.
