Clinical Responsibilities:
Provide comprehensive outpatient primary care to underserved populations including low-income, rural, homeless, and otherwise marginalized individuals.
Perform patient assessment, diagnosis, and treatment of acute and chronic conditions across the lifespan.
Order and interpret diagnostic tests, prescribe medications, and develop individualized treatment plans.
Delivery of care consistent with Aviva Health policies, procedures, and quality standards, emphasizing prevention and health promotion.
Document encounters in a timely and accurate manner in the electronic health record.
Respond to medical questions and provide patient education regarding medications, health conditions, and preventive care.
Collaborate with an interdisciplinary care team including medical assistants, nurses, behavioral health, pharmacy, and dental teams.
Participate in staff meetings, continuing education, and maintain required licensure and certifications.
Uphold Aviva Healths mission, vision, and values in all patient and staff interactions.
Qualifications:
Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Oregon.
DEA certification and BLS required.
One-year professional Advanced Practitioner experience in a clinic setting.
Demonstrated commitment to underserved populations and team-based care.
Strong interpersonal, communication, mentoring, and organizational skills.
Ability to work independently and collaboratively in a fast-paced environment.
Has knowledge and skills to provide services within scope of practice which may include the following age specific groups:
Neonates (birth to 1 year)
Children (1 year to 12 years)
