Social Worker - Care Coordination - Hudson Valley - Full Time - Days
At NewYork Presbyterian Hudson Valley Hospital (located in Cortlandt Manor, Upper Westchester County, NY), talented Social Workers lead, learn, and educate in a department like no other. You will serve as a highly valued and respected leader from day one, empowered to make amazing things happen for our patients and our growing success as one of the members of the NewYork-Presbyterian Regional Hospital network.
You will manage complex cases and serve as a strong and knowledgeable liaison between acute care and critical care teams, coordinating the efforts of all disciplines in support of patients and their families. Direct all aspects of inpatient care, ensuring optimal length of stay, anticipate outpatient service needs, and set follow-up appointments, consultations as needed, and other components necessary in the discharge planning process.
This is position is M-F 9a-5p at NewYork-Presbyterian Hudson Valley Hospital (located at Cortlandt Manor, Westchester County, NY) with rotating weekend requirements. Position may cover the intensive care, maternal child health, inpatient medicine units, Emergency Department, and/or PACU.
