Nurse
The Dutchess Reintegration Forensic ACT team (Assertive Community Treatment team) is a multidisciplinary team including clinicians, nurses, prescriber(s), and case managers. The FACT team is working at the forefront of the intersection between behavioral health and involvement in the criminal justice system. Our mission is to support our participants through innovative services that improve behavioral health, promote recovery and rehabilitation, create community success, and reduce incarceration. Services are delivered in the person’s home or community, available 24/7, and focus on helping individuals stay stable, avoid hospitalization, and build skills for independent living.
The FACT Team Nurse is an essential member of the multidisciplinary Assertive Community Treatment (ACT) team, responsible for providing nursing care, health education, medication management support, and clinical coordination to individuals with severe and persistent mental illness. The nurse delivers services primarily in community settings—clients’ homes, shelters, and other natural environments—to support stability, recovery, and wellness.

Work Environment:
This position requires frequent community-based work, travel within the service area, and interaction with clients experiencing significant clinical and social complexity. Flexibility, strong boundaries, and sound clinical judgment are essential.
Successful completion of background check including Justice Center and New York State Office of Children and Family Services Statewide Central Register Database Check.
